Please look up your Veterinarian’s phone number and include it on the form. We are all volunteers with families and work. Omitting important numbers slows us down the process.

1. Dogs 2nd Chance needs permission to verify your vaccination history and Heartworm preventative purchases.
2. If you use alternative heartworm prevention methods, please note in the heartworm brand section.

If you are a Renter, please contact your landlord and give Dogs 2nd Chance permission to talk with them about any dogs restrictions.
